The National Transportation Safety Board's final report attributed the incident to pilot error after eight skydivers exited the balloon safely. Van der Walt opened the rapid deflation valve, leading to a rapid descent that ended in a crash. Despite his efforts to add hot air and regain control, the balloon continued to deflate. Three passengers died alongside van der Walt, and one other was seriously injured.

The balloon, manufactured in 2011 and operated by Droplyne Hot Air Balloon Rides, had no mechanical issues prior to the crash. Investigators noted that if van der Walt had used the parachute valve, he could have landed safely. This incident highlights the importance of adhering to operational protocols during balloon flights.
